You can search for a NAME RECORD by either entering the Certification ID or by using a partial last name search.

Enter the person's certification ID in the form at right using a nine digit SSN for US records with no spaces, dashes, etc. Use the appropriate 6 or 7 character ID for Canada records. Click the "SUMBIT ID" button to process the record search.

If you do not have a Certification ID, enter a partial last name value, such as "SMIT" for Smith, Smithson, etc. Do not include any name value other than for the last name to assure a valid search result.

A list of matches will be displayed. You can see details for any of the found records by clicking on the name of any listing.

Click on the "CANCEL" link above and to the right of the list to return to the record search page. The found record is displayed with all information including the Clearinghouse Actions and Other Names.

Click on an action description for more detailed information. Click on the "Return to List" link to return to the record and list of actions. Click on the "Search" link to return to the record search page.
